Behavioral Health

We provide a wide range of services for patients experiencing acute episodes or exacerbation of mental illness, in their homes where they feel most comfortable.

Holistic care for both mind and body

Behavioral health is important for everyone’s well-being, and we address it via a multidisciplinary team that considers the whole person when creating an individualized plan to address their mental health symptoms. By providing a wide range of services to address these needs, we make sure patients get the support they need at home.

What to Expect

What you might encounter with mental health nursing in the privacy of your home:

Assessment

At the start of our work with you, we’ll conduct a series of assessments. This might include any or all of the following:
1) Assessment of your mental status, level of functioning, coping skills and home environment safety
2) Assessment of your home safety environment during outpatient electroconvulsive therapy (ECT)
3) Assessment for any additional services you may need, including medical home health or community programs

Education

This includes processing disease diagnoses and illness management for you and your regular caregivers, as well as medication education and assessment.

Stress Management

We’ll work with you to implement stress management techniques, communication skills, coping mechanisms and behavior modifications.

Administration and Monitoring

As part of this work, we’ll often provide administration of intramuscular decanoates with close monitoring for adverse reactions.

Transitions

When a transition occurs, it’s often difficult and emotional. During these times, we’ll provide assistance with your transfer to a more appropriate home setting if necessary, such as an apartment, assisted living facility, retirement center or residential care facility.

Do I qualify?

In addition to our general admission requirements for VNA services, there are specific requirements for mental health nursing services. These include:

  • You must have an acute (short-term) problem or a chronic problem with acute exacerbation.
  • You must cooperate with the plan of care and the VNA Home Health mental health team.
  • You must not pose a threat to self or others.

Please note: There are some homebound exceptions for mental health home care include:

  • May not have physical limitations.
  • Illness prevents ability to leave home at all or requires assistance by another person for emotional support and/or coping management.
