Advanced Illness Management

A palliative care program that prepares patients and caregivers for transitions related to severe chronic illness whether the outcome is a return to more fully independent living or a transition to end-of-life care.

Education for patients and caregivers

Advanced Illness Management (AIM) is a VNA Home Health program that employs a team of skilled professionals trained in care coordination and transitions, as well as providing self-management education for the patient with advanced illness and their caregivers.

Every VNA Home Health team member is a committed caring professional who has experience, knowledge and training to help you manage your complex condition. When given skilled, respectful coaching and care, patients can better manage their health.  The focus of care for advanced illness is moved out of the hospital and into the home and the community.

Types of conditions

Our program addresses the specific education needs of patients who have been diagnosed with one or more of these chronic conditions:

Chronic Obstructive Pulmonary Disease (COPD)

Congestive Heart Failure (CHF)

Diabetes

Coronary Artery Bypass Graft/Myocardial Infarction (CABG/MI)
