Our occupational therapists work with patients to make sure they get the tools and support they need to live independently in and outside their home. Whether the impairment is permanent or temporary, we are able to provide care that goes above and beyond.

What to Expect

VNA’s Occupational Therapy uses everyday life activities to promote health, well-being and your ability to participate in the important activities in your life.

Evaluation

First, we’ll conduct an evaluation that assesses your history, life experiences, goals and interests. This will help us better understand your values and the activities that are important to you.

Action Plan

Together, we will use what we learn from your evaluation to identify your goals for our program and begin to create an action plan toward achieving key milestones.

Regular Check-ins

As part of your action plan, we’ll identify key milestones to work toward as well as when to check in. During those check-ins, we’ll talk about your progress as well as make any changes to the plan in order to meet those key milestones.

Ongoing Work After Discharge

Eventually, you will be discharged from our care, but that doesn’t mean that the work ends. We’ll leave you with activities and exercises to work on to support your long-term health and wellness.

What do I need to know about Occupational Therapy?

Can VNA provide services at home if a patient is already receiving outpatient services?

No, we cannot provide services at home if the patient is being provided outpatient services as well, unless it is a service that we cannot provide such as whirlpool therapy.

What tools can Occupational Therapists help patients secure? 

Some patients require adaptive equipment depending on their condition, and our therapists can help patients pick out what is necessary to complete tasks they may have troubles with. Our therapists will assess patients’ needs and guide them in the purchase of those tools, which they will then use in their treatment.

What sorts of impairments can Occupational Therapists help with?

We provide occupational impairments to any patient with a permanent or temporary impairment that diminishes physical or mental abilities to perform routine tasks around the home.
