What services can skilled nurses provide at home in addition to direct patient care?

- Initial in-home assessments to determine services needed and develop a plan of care (upon request)
- Ongoing communication and coordination with primary care providers
- Regular reevaluation of patient needs and adjustments to the plan of care as needed
- Educational guidance for the patient and family or regular caregiver

What do initial in-home assessments consist of?

Initial assessments not only make sure of our patients’ physical needs but also make sure they comply with the rules and regulations required under insurance or Medicare. It is a full medical assessment to make sure we understand all their needs, even beyond skilled nursing, so that they are treated holistically.

What does the partnership between a patient’s primary care provider and VNA’s nurses look like?

We always like to say the nurses are the eyes and ears of the physician in the home. The physician has ordered us to assess and treat these patients, so our responsibility is very collaborative to bring in the information the physicians need and advocate for the patients.

How does a patient get started with skilled nursing?

Our Skilled Nursing Services for Hospice include: pain management, ostomy care, port access and maintenance, foley catheter care, wound care, trach care, and drain management.Skilled nurses must be ordered by a physician with a physician signing orders. There has to be a skill to be provided by the nurse; we cannot see patients who require no skill except bathing services.
