Social services address a patient’s physical, emotional and economic needs in partnership with Masters in Social Work.
Addressing emotional and mental health in home care
Individuals in our home health services will have the option to be paired with clinical Social Worker who specializes in mental and behavioral health. Among many things, the core of their services are to meet the patient where they are, understanding both their emotion and physical needs, as well as their regular home routines and overarching goals.
These services includes one-on-one education about health and wellness, behavioral health and mental stability assessments and counseling for emotional or mental health concerns. Additionally, they can provide support with coping skills and anxiety management.
General Social Work Services
At VNA, we understand that healthcare is more than just the medical diagnosis of care, but rather it’s a look at the health and well-being of the entire patient.
At VNA, we believe that there are many facets of healthcare, from housing benefits to food assistance, to help patients improve in overall health and wellness.
Medical Social Work
VNA’s medical-specific social work focuses greatly on healthcare coordination and supporting patients as they manage economic and/or emotional factors related to illness, injury or plans of care.

One component of our Social Services is Behavioral Health services as people navigate change, transition and the many emotions that accompany a disease diagnosis.
